Secondary Functions:
Under the supervision of the Home Health Service Director/Physical Therapy Coordinator, uses skills and modalities of professional P.T.; evaluation, assessment, care planning and periodic reassessment of individual Plans of Treatment to enable homebound patients to obtain optimal levels of physical independence according to age and/or developmental ability. Coordinates services with other team members. Teaches and supervises Home Health Aides in simple P.T. skills. Devises forms and written instructions as needed.
