Provide RNP care under MD supervision, triage, histories, physical assessment, crisis management and follow-up.
Essential Functions:
Direct Patient Care Peforms:
- Initial and ongoing medical, functional, and psychosocial assessments of patients and families on a timely, appropriate basis.
- Discusses patient needs and desires with the patient and family, and uses them as a factor in the life care path, interventions, and treatment.
-Participates in the development of the life care path and clinical maps (paths) for patients based on patient assessment and team conferences, and ensures effective implementation.
- Provides direct patient care and services; including but not limited to: medications, including furnishing, treatments, patient education, and appropriate follow-up/referral.
- Monitors the patient's progress, as needed and as appropriate.
- Documents patient care in a timely, accurate, and concise manner.
- Shares responsibility for telephone triage for all Behavioral Health Clinic patients.
- Provides crisis intervention, as needed.
- Indentifies and coordinates the learning needs of patients and families.
- Acts as a patient/family/caregiver advocate.
- Establishes priorities for patient care based on essential patient needs and available resources of personnel, equipment, and supplies.
- Evaluates effectiveness of care given by all team members.
Care Coordination:
- Reviews patient charts to assist in the patient selection process.
- Collects, enters, and uses data from past/current utilization, the medical record, and the patient and family to help determine patient care needs; evaluates and makes decisions based on this data.
- Identifies gaps in the health care delivery system and ensures the patient and family receive necessary services.
- Comprehensively documents all patient and family activity as it applies to the patient's overall health, safety, and emotional well-being.
- Participates actively in all team conferences related to the nurse practitioners assigned patient load.
Program Coordination:
- Identifies, integrates and coordinates Kaiser Permanente and community health care resources and services for patients and their families/caregivers.
- Participates in educational opportunities to enhance learning for self and others.
- Acts as a consultant/resource to medical center staff.
- Develop, review, and update policies and procedures, protocols, and care paths.
- Participate in quality assurance and improvement.
- Responsible for other duties, as indicated by either the physician or the person's direct supervisor.
- Supervise the work of the Medical Assistant, as appropriate.
- Consistently supports compliance and the Principles of Responsibility (KP's code of conduct) by maintaining confidentiality, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable Federal and State laws and regulations, accreditation and licensure requirements, and KP policies and procedures.

Qualifications:
Basic Qualifications:
-Graduate of accredited Nurse Practitioner Program.
- Master's degree in Nursing required
- Registered Nurse Practitioner.
- Practitioner program or equivalent required
- A new graduate Nurse Practitioner or newly employed Nurse Practitioner with less than twelve (12) months experience as a Nurse Practitioner.
- NP I shall practicipate in a Nurse Practitiioner mentoring program within the first six (6) months of KP employment.
- Must be Medicare and Medi-Cal eligible which requires a Masters Degree in Nursing and current National Certification from one of the following organizations: American Academy of Nurse Practitioners; American Nurses Credentialing Center; National Certification Corporation for Obstetric, Gynecologic and Neonatal Nursing Specialties; National Certification Board of Pediatric Nurse Practitioners and Nurses.
- Bilingual skills preferred.
- Legible handwriting and good record keeping qualities required.
- Good oral and written English skills required.
-Demonstrated good interpersonal relationship skills required.
- Must be able to interact effectively with patients and co-workers, in a courteous manner.
- Must demonstrate good working relations with peers, physicians, and other staff both at Kaiser Permanente and community agencies.
- Ability to work independently from approved protocols.
- Must be flexible and willing to work with schedule and hours, to meet the needs of the department.
- Ability to work in a Labor Management Partnership environment.
- Furnishing license required within 6 months from hire.
- Current California RNP license required.
- BCLS certification required.
- IV certification.
-Must have a UPIN or be UPIN eligible which requires a Masters Degree in Nursing and current National Certification from one of the following organizations: American Academy of Nurse Practitioners; American Nurses Credentialing Center; National Certification Corporation for Obstetric, Gynecologic and Neonatal Nursing Specialties; National Certification Board of Pediatric Nurse Practitioners and Nurses.
-Must have a Nat'l Provider Identifier (NPI, or obtain an NPI, prior toemployment start date.
Preferred Qualifications:
Two years recent (within the last 3 years) experience in inpatient psychiatric as a nurse
Schedule: Scheduled days and hours are dependent upon departmental needs and may include holidays and weekends.
